Volunteer Risk Management Portal
All board members, coaches, assistant coaches and volunteers must complete the online Risk Management form. Click on the link which will take you to the GotSport coaches login page then select Background Check from the menu bar.
If you do not have a user account or password please email your name and address to dennyeby@hotmail.com. You will be sent a username and password to login and complete the required information.

Please download and complete the Game Roster for each game. The form should be given to the referee prior to each game. This form can also be used for games in Kilgore.

1. Click on the HSA Player Registration Form below and print out the registration form. (.pdf format)
2. Click on the HSA Medical Release Form and print it. (.pdf format)
3. Complete the Registration Form and the Medical Form. Please make sure to SIGN the Liability Waiver and Medical Release Sections.
4. Bring form with you to one of the ON-SITE REGISTRATIONS or follow the instructions below for MAIL-IN REGISTRATION.
